Gland, fat, or time to see your GP

Nine short questions for men 18 and over. You'll see the likely answer and the sensible next step. Nothing is sent unless you choose to.

Gland or fat

Not every big chest is gynaecomastia

True gynaecomastia is breast gland: a firm, rubbery disc right behind the nipple. Chest fat is soft all over. They're treated differently, and many men have some of each.

When to see a GP

Gynaecomastia is almost always harmless. A GP should still examine it once in an adult, to find the cause and rule out the rare lump that is something else.

Book a routine appointment if

  • you are over 18 and it is new, or getting bigger
  • it is on one side only
  • it is painful and the pain is not easing
  • a teenager still has it after two years, or past 17
  • it is affecting how you feel or what you do

What the GP will do

Go soon if you notice

  • a hard lump, or a lump that is off to one side rather than centred under the nipple
  • a lump that is fixed and doesn't move under the skin
  • fluid or blood coming from the nipple
  • a nipple that has turned inwards, or skin that is dimpled, red or sore
  • a swelling or lump in your armpit
  • a change on one side only that is getting bigger

Questions

Questions men ask

What is gynaecomastia?

Gynaecomastia is breast gland tissue growing in a man or boy. It shows as a firm, rubbery disc under the nipple, on one or both sides, and it is often tender at first. It is not the same as chest fat, and it is very common: about half of teenage boys get some during puberty.

Is gynaecomastia the same as man boobs?

Not always. "Man boobs" is used for both true gynaecomastia (gland) and chest fat. Gland feels like a firm disc behind the nipple; fat is soft all over. Many men have a mix of the two.

Will it go away on its own?

Often, yes. In teenagers it usually settles within six months to two years. In adults it often settles once the cause is dealt with, such as a medicine, steroids or weight. Tissue that has been there for more than a year tends to turn fibrous and stays.

Can exercise get rid of gynaecomastia?

Exercise burns fat, so it helps when the chest is mostly fat. It can't shrink gland tissue, and chest workouts build muscle underneath, which can push the disc forward. If the firm disc is still there after you're lean, exercise won't move it.

When should I see a GP?

When it's new and you're an adult, when it's on one side, when it's painful, or when it's growing. See a GP soon for a hard or fixed lump, nipple discharge or blood, a pulled-in nipple, skin changes or a lump in the armpit.

Is surgery the answer?

For long-standing gland tissue in an adult whose weight is steady and whose GP has checked for a cause, yes, surgery is the treatment that removes it. In Ireland it costs €3,000 to €6,900 privately, our price guide. It isn't for teenagers who are still growing.

Who will contact me if I use the check?

Nobody, unless you choose to send it. If you do, one Irish clinic contacts you to arrange a consultation with a surgeon. Nothing is booked until you say yes.
